At Kirkby Community Primary Care Centre, we operate an online system for booking appointments, prescription requests and medical advice.
Dr.iQ is now powered by Evergreen. This means you can now request appointments with us online via the Evergreen Life app.

If you’re unable to download the app or don’t want to use online services, please contact the practice directly by phone to make a request.
We provide a wide range of services. There are a range of professionals who can look after you. You may be offered an appointment with an advanced nurse practitioner, physiotherapist, paramedic or clinical pharmacist rather than a GP.
This appointment may take place face-to-face at the practice, on the telephone or via video call.
Before you make an appointment to see your GP, think about what other services might be able to help.
You do not have to make an appointment or register for these services.

We understand that sometimes you may not be able to attend your booked appointment. If you cannot attend, help us by letting us know as far in advance that you cannot make your appointment. This means we can offer the appointment to another patient who needs it.

Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.
You do not need your NHS number to use NHS services.
You should have an NHS number if you registered for NHS care in England, Wales or the Isle of Man.

What is Pharmacy First?
Pharmacy First is an NHS service that allows patients to receive advice, assessment and, where appropriate, NHS-funded treatment for several common health conditions directly from a community pharmacy, without needing to see a GP first. The service is designed to provide quicker access to care and help free up GP appointments for patients with more complex needs.

We are able to visit very poorly and housebound patients in their homes. If you need a home visit please call the surgery by 10am so the Doctor can plan their rounds. Home visits are normally made between 12.30pm and 3.30pm.
You can view your medical record using the Dr. iQ powered by Evergreen Life app or NHS app. The medical record feature gives you access to information such as: test results, medications, documents and letters, immunisation history, consultation history and allergy information.
